Mass General Brigham is seeking a skilled Registered Nurse to join our team as a Case Manager. This is a unique opportunity to work in a multidisciplinary, cross-institutional team that plays a central role in managing capacity on behalf of the eleven hospital MGB system.
About the Role:- As a Case Manager, you will work closely with teammates within the PTAC, colleagues at each member hospital, and other key stakeholders across the system to enable safe, efficient, and timely patient care.
- You will serve as the chief manager for the Repatriation program, which involves identifying candidates among patients transferred to or within MGB, preparing and sending hospital teams for Repatriation, and tracking patients during their hospital stay.
- You will also work to strengthen system-wide capabilities that optimize patient inflow and outflow at MGB hospitals and other MGB assets.
- Monitor system-wide capacity and develop strong knowledge of hospital and non-hospital assets throughout MGB as well as capabilities at non-MGB facilities.
- Identify and manage scenarios in which a patient may benefit from alternative levels of care or alternate placement, including out-of-network transfer requests and out-of-state transfer requests with anticipated difficult disposition.
- Work closely with colleagues in the Specialty Desks within the MGB PTAC to help address barriers to discharge that are escalated from the MGB hospitals or other assets.
- Registered Nurse (RN) licensure and experience in case management or a related field.
- Strong communication and collaboration skills, with the ability to work effectively with diverse stakeholders.
- Ability to analyze complex data and make informed decisions.
