About the Role

We are seeking a skilled RN Case Manager to join our team at ChristianaCare Direct. As a RN Case Manager, you will play a critical role in coordinating patient care and driving patient progression to establish a discharge plan.

Key Responsibilities
  • Coordinate care and develop discharge plans for patients with post-acute care needs
  • Collaborate with interdisciplinary teams, including physicians, nurses, and social workers, to ensure comprehensive care
  • Identify and address system issues that serve as barriers to care

  • Case Management Certification (CCM) required within 18 months of eligibility
