Licensed Practical Nurse

Mass General Brigham Healthcare at Home is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality nursing care to patients in their homes. As a member of our interdisciplinary team, you will work collaboratively with patients, families, and healthcare professionals to deliver patient-centered care that meets the unique needs of each individual.
Responsibilities- Provide skilled nursing visits to patients in their homes, utilizing thorough and timely electronic documentation and interdisciplinary communications.
- Collaborate with the RN case manager to develop and implement individualized care plans that address the physical, emotional, and social needs of patients.
- Conduct thorough assessments of patients' physical and emotional status, identifying areas of concern and developing strategies to address them.
- Administer medications and treatments as prescribed by physicians, and monitor patients' responses to these interventions.
- Provide education and support to patients and their families on disease management, medication use, and other health-related topics.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies to improve patient outcomes.
- LPN professional license in Massachusetts.
- Minimum 1 year previous nursing experience in acute care or sub-acute care setting required.
- Home care experience preferred.
- Effective written, verbal, and electronic communication skills.
- Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred.
