Responsibilities
  • Provide diversified clinical duties in providing skilled nursing care to acutely ill or chronic patients in the home.
  • Assess patients' needs and develop individualized care plans to ensure optimal outcomes.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making.
  • Participate in ongoing education and professional development to stay current with best practices and industry advancements.
Requirements
  • Current licensure as a Registered Nurse in the state of Massachusetts.
  • Competency in basic nursing skills and excellent assessment skills.
  • Ability to speak, read, and write English.
  • Per diem position with a minimum of 2 weekend shifts per month and 1 holiday per year.
