Pathway of Hope Case Manager

Classification: Full-Time, 40 hours per week, 8:30 a.m. – 5:00 p.m.
The Pathway of Hope Case Manager plays a vital role in providing direct service provision to clients within the POH program. This innovative, holistic, client-centered approach aims to help families break cycles of intergenerational and chronic poverty by achieving goals through intensive case management and experiences in flourishing, supportive relationships.
Key Responsibilities:
- Empower families to meet their goals and objectives.
- Collaborate with clients to identify and address their needs.
- Develop and implement personalized case plans to promote family stability and self-sufficiency.
- Provide support and guidance to clients in achieving their goals.
Qualifications:
- An understanding of and passion for The Salvation Army's Mission.
- Bachelor's degree in Social Work strongly preferred, related degrees will be considered.
- Licensed Social worker in Ohio, strongly preferred.
- Experience and/or strong interest in community outreach, organization, and community capacity development.
- Ability to work with people and clients of diverse racial, ethnic, and socio-economic backgrounds in a sensitive and culturally appropriate manner.
