Position Overview:

As a Registered Nurse Care Coordinator, you will play a vital role in providing compassionate care to patients in need. At Interim HealthCare, we prioritize a holistic approach to patient services, ensuring comfort and dignity for all our clients.

Your Responsibilities:
As a Registered Nurse Care Coordinator, your duties will include:
  • Conducting in-person patient interviews and thorough physical evaluations.
  • Overseeing the execution and continuous evaluation of patient care plans by managing home health aides, LPNs, RNs, and other caregivers.
  • Communicating patient conditions effectively and collaborating with healthcare providers to adapt care as patient needs change.
  • Educating patients and their families on essential home health care practices, including wound care, IV administration, and medication management.
  • Working proactively to reduce hospital readmissions by encouraging preventive care and ensuring seamless continuity of care.

Qualifications:
To be considered for the Registered Nurse Care Coordinator position, you must possess:
  • A current, unrestricted license to practice as a Registered Nurse (RN) in the relevant state.
  • Current CPR/AED/BLS/First Aid certification.
  • Reliable transportation for travel to care sites.
  • A minimum of one (1) year of professional nursing experience in home health or a related field; prior case management or utilization review experience is preferred.
  • Experience with OASIS documentation is preferred.
  • Practical experience with tracheostomy and/or ventilator care is advantageous but not mandatory.
