Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Medi Home Health Agency. As a key member of our multidisciplinary team, you will provide high-quality patient care to our clients in the comfort of their own homes.

Responsibilities
  • Deliver skilled nursing care to clients under the direction of a registered nurse and physician's plan of care.
  • Assist in planning and exchanging information with other health professionals to ensure continuity of client care.
  • Prepare clinical progress notes and follow client care plans as developed by the registered nurse and according to agency policy.
  • Participate in on-call rotation to provide 24/7 support to our clients.
Requirements
  • Successful completion of a certified licensed practical nursing program.
  • Current licensure as an LPN in the state of PA.
  • Minimum of one year of nursing experience, with home health experience strongly preferred.
  • Reliable transportation and company required auto liability insurance.
