Job Summary

We are seeking a skilled LPN Scheduler to join our team at Medi Home Health Agency. As a key member of our home health team, you will be responsible for scheduling admissions, initial evaluations, and home health aide visits in a timely and efficient manner.

Key Responsibilities:

  • Receive notifications for patients to be scheduled and review visit frequency to determine necessary disciplines.
  • Review staff scheduling to assess availability and schedule first visits for licensed staff.
  • Consult with Clinical Manager as needed to locate staff to cover visits.
  • Update assignment screen based on scheduling and contact staff members by phone to notify them of same-day visits.
  • Ensure home health aide visits are scheduled for the entire ordered visit frequency and prepare weekend visit schedules.

Requirements:

  • Successful completion of a Certified Licensed Practical Nursing program required.
  • Currently licensed as an LPN in the state of PA.
  • Home Health clinical experience strongly preferred.
