Key Responsibilities- Conduct thorough home visits and telephonic case management to assess patient needs and develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Provide education and support to patients and their families on disease management, pain and symptom control, and end-of-life care.
- Initiate and implement preventive and rehabilitative nursing procedures as needed.
- Document patient progress and outcomes in the electronic health record.
- Participate in ongoing education and training to stay up-to-date on the latest clinical practices and guidelines.
- Current RN licensure in the state of employment.
- 3+ years of experience in a clinical or hospice setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
- Hospice and Palliative Care Nursing certification.
- Experience with home visits and community-based nursing.
- Knowledge of palliative and hospice medicine.
- Bilingual (English/Spanish) language proficiency.
