Job Summary

We are seeking a Full-time Hospice Registered Nurse Case Manager to join our team in Wichita, KS. As a member of our interdisciplinary team, you will work under the direction of the Director of Nursing, attending physicians, and the interdisciplinary group to provide skilled nursing assessments, planning, and care. Your primary responsibility will be to assess, plan, implement, and evaluate the nursing process for a patient caseload.

Key Responsibilities
  • Assess patient needs and provide supportive care in accordance with attending physician orders and the plan of care.
  • Communicate effectively with attending physicians, Hospice staff, and other agencies to coordinate optimal care and resource utilization.
  • Document patient information accurately and timely in electronic medical records.
  • Participate in Interdisciplinary Group meetings and contribute to patient care planning.
  • Supervise and document the NAC every two weeks or as assigned.
  • Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues.
  • Assume the role of associate nurse during on-call time or as requested by the Patient Care Coordinator.
  • Provide support at time of death and bereavement.
Requirements
  • Current license to practice as a registered nurse in the State of KS.
  • At least two years of work experience as a professional nurse, preferably in hospice care.
  • Ability to assess and respond to patient needs in varied settings.
  • Skilled in nursing practice, able to cope with emotional stress, and tolerant of individual lifestyles.
  • Ability to make meaningful observations, skilled assessments, and write comprehensive reports.
  • Must be a team player who supports an interdisciplinary team concept of caring for patients and their loved ones.
  • Valid driver's license, good driving record, reliable vehicle, and ability to drive year-round to locations.
