About the Role
We are hiring skilled LPNs to work in Long Term Care/Assisted Living Facilities. In this role, you will be responsible for delivering high-quality patient care, making clinical judgments, and coordinating care with the medical staff. Your expertise will include assessing patient needs, developing plans of care, and evaluating patient outcomes.

Key Responsibilities
  • Utilize the nursing process to assess, plan, implement, and evaluate patient care
  • Perform focused interviews to identify specific patient needs
  • Collect, analyze, and interpret data from healthcare members and document actual and potential nursing diagnoses

Requirements
To be successful in this role, you will need:
  • A current LPN license for the state where you practice
  • Strong communication skills, both written and verbal
  • One year of prior professional nursing experience, preferably in Long Term Care
  • A current BLS card
