BAYADA Home Health Care is seeking a skilled Home Health Physical Therapy Assistant to join our team in Shelburne, Vermont.

Job Summary:

We are looking for a dedicated and compassionate Physical Therapist Assistant to provide high-quality care to our clients in the comfort of their own homes. As a Home Health Physical Therapy Assistant, you will work closely with our licensed Physical Therapists to develop and implement personalized treatment plans that promote optimal physical well-being and independence.

Responsibilities:
  • Assist clients in achieving their physical therapy goals through various interventions, including exercises, modalities, and education.
  • Conduct thorough assessments and evaluations to identify client needs and develop effective treatment plans.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to clients and their families to promote self-care and independence.
  • Maintain accurate and timely documentation of client progress and treatment plans.
Requirements:
  • Current licensure or certification in the state of Vermont.
  • Graduation from a two-year college-level program approved by the American Physical Therapy Association.
  • At least one year of experience as a Physical Therapist Assistant under the supervision of a qualified Physical Therapist.
