Disability Support Specialist

About the Role:
">Catholic Charities Family and Community Services is seeking a skilled Disability Support Specialist to join our team. As a Disability Support Specialist, you will work directly with individuals who have chosen to self-direct their services, providing support and assistance to help them achieve their goals.
">Key Responsibilities:

- Develop and implement individualized plans to meet the unique needs and preferences of each person served.">
- Provide facilitative outcomes, such as personal care, community integration, money management, advocacy, employment needs, and respite, as described in each individual's Self-Directed Plan.">
- Communicate effectively with individuals, families, and caregivers to ensure seamless support.">
- Maintain accurate records of contacts with individuals, including progress notes and incident reports.">
- Attend mandatory training sessions to stay up-to-date on best practices and industry developments.">
Requirements:
">A high school diploma or equivalent is required, with one year of practical experience working with individuals with intellectual/developmental disabilities preferred. A combination of education and experience may be substituted at the discretion of the Executive Director.
">The estimated salary for this position is $45,000 - $55,000 per year, depending on experience and qualifications.
