Primary Care Nurse Practitioner

Job Summary: We are seeking a highly skilled and compassionate Primary Care Provider, Nurse Practitioner to join our team at Kintegra Health. As a key member of our healthcare team, you will provide direct and indirect care to assigned Senior TLC participants in a clinic, home, or community-based model of care.
Key Responsibilities:
- Perform health histories, physical exams, and patient teaching.
- Order, interpret, and follow up on lab tests and results.
- Perform selected procedures and document care orally and in writing.
- Initiate emergency measures as indicated.
- Recognize abnormalities that justify immediate referral for intervention by others.
- Facilitate participant primary care management in conjunction with physician staff as is reflective of the PACE Model.
- Initiate, evaluate, review, and change orders for selected medications and diagnostic studies/treatments within attending physician approved protocols.
- Complete medication reconciliations on a ppt's admission to a Skilled Nursing Facility (SNF) and reconcile medications at SNF per STLC standard protocol.
- Assess, initiate, and coordinate direct participant care daily under the supervision of the Medical Director and/or Primary Care Physician.
- Perform assessments for PACE participants as required by PACE regulations.
- Visit participants in homes and hospitals as directed/required.
- Request consultations from other services as needed in collaboration with attending physician and/or established protocols.
- Collaborate with physicians, nurses, and other healthcare professionals, providing education, communication, and support for healthcare staff, patients, and families.
- Participate in participant care planning, including the implementation of SMART goals/interventions for the participant's plan of care and enter all care plan information in a timely manner as per organizational protocols.
Requirements:
- Master's Degree, or greater, as a Nurse Practitioner (NP) from an accredited program.
- 3-5 years medical experience with 1 year's experience as a Nurse Practitioner; and one year of experience with a frail or elderly population.
- NP licensed in NC and Board Certification (ANCC).
Working Conditions: Must be medically cleared for all communicable diseases and have proof that all immunizations are up to date before engaging in direct participant contact.
