Job Overview
Hospice RN Case Manager - California (Inland Empire, Riverside County, and Temecula Area)

We are seeking an experienced Registered Nurse (RN) to join our team as a Hospice RN Case Manager. As a hospice nurse care coordinator, you will be responsible for admitting and leading the coordination of patient care with the multi-disciplinary team.

About the Job
The Hospice RN Case Manager is an integral component of our dedicated and compassionate team of nursing and hospice professionals. At Tranquil Oaks Hospice Care, patients always come first.

Salary and Benefits
The estimated salary for this position is $95,000 - $115,000 per year, depending on experience. In addition to a competitive salary, we offer comprehensive benefits, including 401(K), health insurance, paid time off, and mileage reimbursement.

Responsibilities
- Evaluate patients for hospice appropriateness
- Develop a detailed plan of care for assigned patients
- Provide comprehensive nursing evaluations and assessments
- Coordinate medications and DME needs for same-day delivery
- Participate in patient care conferences and supervises Home Health Aides assigned

Requirements
- Graduate from an accredited school of nursing
- Currently licensed as a registered nurse to practice in the State of California
- Minimum 1 year of experience as a professional nurse within the last three years
- Previous experience in hospice, home health, admissions preferred
