Key Responsibilities
  • Conduct medical nutrition therapy assessments for residents to address both acute and chronic health issues.
  • Provide education on therapeutic diets and nutritional interventions aimed at improving health outcomes and quality of life.
  • Collaborate with nursing staff, dietary managers, speech-language pathologists, physicians, and nurse practitioners to deliver tailored nutritional therapy.
  • Monitor residents experiencing weight fluctuations, enteral feeding needs, dialysis, and wound care.
  • Engage in resident review meetings to discuss care plans for high-risk individuals.
  • Perform nutrition assessments across a diverse patient population.
  • Develop and implement nutrition intervention plans in line with the Nutrition Care Process.
  • Offer personalized nutrition counseling, adapting teaching methods to meet individual learning needs.
  • Utilize assessment techniques that consider age-specific and cultural needs.
  • Ensure appropriate follow-ups aligned with treatment goals, referring residents to outpatient counseling or community services as necessary.
  • Work in tandem with medical and food service staff to ensure adherence to prescribed nutrition care orders.
  • Educate staff on food safety, sanitation, and workplace safety standards.
  • Document nutrition care plans thoroughly in residents' medical records, detailing assessments, diagnoses, plans, and progress.
  • Maintain confidentiality regarding patients' protected health information.
  • At Aramark, we encourage skill development and adaptability to meet job requirements, which may evolve over time.
Qualifications
  • Bachelor's degree required; Master's degree preferred in Dietetics or a related field.
  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential is necessary.
  • Illinois state licensure as a Dietitian is required.
  • Familiarity with PointClickCare and MDS processes is advantageous.
