Job Summary

The Addiction Medicine Advanced Practitioner Training Program at Geisinger Health System is designed to provide integrated mental health and substance use disorder (SUD/OUD) services in a primary care underserved community-based setting. The program aims to foster robust clinical training and augment expertise among clinicians who will see patients at access points of care and provide mental health and addiction prevention, treatment, and recovery.

Key Responsibilities
  • Participate in Medications for Opioid Use Disorder (MOUD) in community-based settings, including professional peer-to-peer support to assist trained advanced practice nurses and physician assistants in initiating their provision of MOUD.
  • Connect and manage patients receiving MOUD, providing behavioral health counseling and connecting them to social supports to retain patients in care.
  • Collaborate with caregivers inside and outside of the immediate care team.
  • Demonstrate effective communication with patients and the immediate care team.
  • Perform initial and ongoing assessment of patients' medical, physical, and psychosocial status.
  • Develop assessment and treatment plans.
  • Participate in clinical patient care.
  • Record progress notes.
  • Order tests, examinations, medications, and therapies.
  • Arrange for discharge and aftercare.
  • Write and dictate admission notes, progress notes, procedure notes, and discharge summaries.
  • Provide patient education and counseling covering health status, test results, disease processes, and discharge planning.
  • Perform procedures.
Requirements
  • Master's Degree in Nurse Practitioner (Required)
  • Certification(s) and License(s): Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania; Certified Nurse Practitioner - American Academy of Nurse Practitioners
