Job Summary

Elevance Health is seeking a skilled and compassionate Nurse Case Manager to join our team. As a Telephonic Nurse Case Manager, you will play a critical role in ensuring our members receive high-quality, patient-centered care.

Key Responsibilities
  • Conduct thorough assessments of members' health needs and develop personalized care plans to optimize their health outcomes.
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination and communication.
  • Provide education and support to members and their families to promote healthy behaviors and self-management skills.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed to ensure optimal outcomes.
  • Document all interactions and care plans in a timely and accurate manner.
Requirements
  • Current, unrestricted RN license in the applicable state(s).
  • Minimum of 3 years of clinical experience, preferably in a hospital or home health setting.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
Preferred Qualifications
  • Experience in case management, care coordination, or a related field.
  • Knowledge of healthcare policies, procedures, and regulations.
  • Ability to work in a virtual environment and utilize technology to communicate with members and healthcare providers.
