Job Summary

SSM Health is seeking a Registered Nurse - Case Manager to join our team. As a RN-Case Manager, you will play a critical role in ensuring patients are discharged from the hospital safely and efficiently.

Key Responsibilities
  • Assess patients at high risk for readmission and develop plans to prevent readmission
  • Collaborate with the interdisciplinary team to ensure patients receive the necessary care and support
  • Ensure compliance with Condition of Participation (CoP) discharge planning
  • Develop and implement discharge plans to next level of care
  • May also be responsible for utilization management process, ED case management process, admission process, and acting as a case manager liaison with post-acute providers
Requirements
  • Graduate of accredited school of nursing or education equivalency for licensing
  • Three years' acute care experience
  • Frequent keyboard use/data entry
  • Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Registered Nurse (RN) - Missouri Division of Professional Registration
  • Registered Nurse (RN) - Oklahoma Board of Nursing (OBN)
