Position Summary:

The Hospice RN will serve as a supportive member of the interdisciplinary team (IDT), focusing on delivering high-quality hospice nursing care to assigned patients. This role does not involve managing a case load.

Work Schedule:

Part-Time: 24 Hours

Night Shift: 11:30 PM - 8:00 AM (3 Days per Week, Weekends Required)

Essential Qualifications:

  • Current RN Licensure in Massachusetts.
  • Current RN Licensure in New Hampshire.
  • Basic Life Support (BLS) certification.
  • Minimum of one (1) year of experience in hospice and home health care.

Key Responsibilities:

  • Deliver compassionate hospice nursing care to patients and families, utilizing principles of palliative care and symptom management.
  • Implement nursing processes and adhere to current standards of nursing practice in providing hospice care.
  • Educate patients and families effectively regarding care plans and available resources.
  • Maintain communication with physicians and care team members regarding any changes in patient conditions.
  • Understand family dynamics and engage effectively with diverse family systems.
  • Advocate for patients and families, ensuring they are informed about all available resources.
  • Conduct nursing visits and provide on-call support as necessary to ensure superior hospice care.
  • Document nursing assessments, interventions, and patient responses in accordance with established procedures.
  • Collaborate with Clinical Manager and Medical Director on performance improvement opportunities.
  • Engage in ongoing education regarding hospice care standards and practices.
  • Participate in community hospice education initiatives.
