Position Summary:

The Hospice RN Community acts as a supportive nursing professional within the interdisciplinary team (IDT), ensuring the provision of quality hospice nursing care to each patient assigned. This role does not involve managing a case load.

Work Schedule:

Part-Time: 24 Hours per week, with shifts from 11:30 PM to 8:00 AM, including weekends.

Key Responsibilities:

  • Deliver hospice nursing care to patients and families, employing contemporary principles of palliative care and symptom management.
  • Utilize the nursing process and adhere to current standards of nursing practice in providing hospice care.
  • Facilitate effective education for patients and families.
  • Maintain communication with physicians and Tufts Medicine Care at Home staff regarding any changes in patient conditions.
  • Understand family dynamics and work effectively within diverse family systems.
  • Serve as an advocate for patients and families, providing information about available resources.
  • Assess the need for additional services accurately.
  • Conduct a sufficient number of nursing visits and on-call triage to ensure superior hospice care.
  • Communicate patient reports and status changes clearly and effectively.
  • Regularly update Care Plans, Medication Profiles, and Treatment Plans as patient conditions evolve.
  • Complete all nursing documentation according to established procedures and standards of practice.
  • Collaborate with the Clinical Manager and Medical Director when necessary.
  • Identify opportunities for performance improvement and actively work to address them.
  • Enhance knowledge of hospice care and standards of practice proactively.
  • Participate in community hospice education initiatives.
