Role Summary:

The Hospice RN Community functions as a support nurse within the interdisciplinary team (IDT), responsible for delivering high-quality hospice nursing care to assigned patients. This role does not involve managing a caseload.

Work Schedule:

Part-Time: 24 Hours

Shifts: 11:30 PM - 8:00 AM (3 Days per Week, Weekends Required)

Key Responsibilities:

  • Deliver hospice nursing care to patients and families, employing current palliative care principles and symptom management techniques.
  • Utilize the nursing process and adhere to established nursing standards in providing hospice care.
  • Engage in effective patient and family education.
  • Communicate changes in patient conditions to physicians and Tufts Medicine Care at Home staff.
  • Understand family dynamics and work effectively within diverse family systems.
  • Act as an advocate for patients and families, providing information about available resources.
  • Assess the need for additional services accurately.
  • Conduct nursing visits and on-call triage to ensure superior hospice care.
  • Provide concise and effective communication regarding patient reports and status changes.
  • Update Care Plans, Medication Profiles, and HHA Treatment Plans as necessary.
  • Complete all nursing documentation in accordance with procedures and nursing standards.
  • Collaborate with the Clinical Manager and Medical Director as appropriate.
  • Identify opportunities for performance improvement and actively work to address them.
  • Enhance knowledge of hospice care and standards of practice.
  • Participate in community hospice education initiatives.
