Position Overview
The Resident Care Coordinator plays a pivotal role in ensuring the highest standards of care for residents. This position involves the development and continuous management of individualized care plans, both preliminary and comprehensive, while adhering to established guidelines.
Key Responsibilities:
- Formulate and maintain resident care plans in collaboration with various professional service disciplines.
- Ensure compliance with facility, state, and federal regulations.
- Uphold the policies and procedures of the Colville Tribal Convalescent Center and the Colville Confederated Tribes.
Qualifications:
Education and Credentials:
- Must possess and maintain a valid Registered Nurse (RN) license in the state of Washington.
- A minimum of two years of nursing experience in a healthcare environment is required.
Skills and Competencies:
- Proficient in basic computer and keyboarding skills.
- Ability to adapt to a flexible work schedule as necessary.
- Strong understanding of nursing and medical protocols within a long-term care context.
- Capable of making independent decisions and demonstrating organizational skills.
- Familiarity with the MDS 3.0 process is essential.
- Must be an organized individual with the capacity to develop and execute resident care plans effectively.
- Ability to work autonomously while also collaborating within an interdisciplinary team.
