We are seeking a highly skilled and dedicated Occupational Therapist Home Health Specialist to join our team at CarePartners Living LLC. As a Primary Case Manager OT HH, you will play a vital role in evaluating patients, developing care plans, and providing therapeutic care to help individuals achieve their maximum potential.
Key Responsibilities- Evaluate patients to identify their needs and develop personalized care plans
- Provide occupational therapy services to patients in their homes, focusing on improving their functional abilities and quality of life
- Supervise and coordinate therapeutic care with other healthcare professionals to ensure comprehensive patient care
- Communicate effectively with patients, families, physicians, and other healthcare staff to ensure seamless care coordination
- Participate in the management of resources for patient care and organizational improvement
- Master's degree in Occupational Therapy
- Current licensure to practice Occupational Therapy in North Carolina
- One to two years of experience in occupational therapy, preferably in a home health setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
