Home Care LPN in White Bear Lake Area Opportunity

Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Care team in White Bear Lake Area. In this rewarding role, you will have the opportunity to make a positive impact on the lives of our patients and their families.
About the Role:
- This is a full-time position scheduled for 8-hour day shifts, with flexibility to consider four 10-hour days if interested.
- You will be responsible for providing home care to patients in White Bear Lake and the surrounding area.
- The successful candidate will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
- A valid driver's license is required upon hire.

Your Key Responsibilities:
- Provide Safe and Quality Patient Care:
- Conduct thorough assessments of patient health status through data collection and comparison.
- Recognize and report changes in patient condition to RN Case Managers or licensed healthcare providers.
- Accountability for the quality of care delivered, recognizing limits of knowledge and experience.
- Collaborate with other healthcare providers to develop and modify patient care plans.
- Clearly document pertinent data, including observations, interventions, and teaching.
- Determine and implement appropriate interventions within a nursing plan of care or when delegated by a Registered Nurse.
- Assist in evaluating responses to interventions.
- Practice all patient experience principles to provide an excellent experience for patients and their families.
- Advocate for patients and families in whatever manner necessary.
- Communicate patient learning needs to RN Case Managers.
- Implement teaching strategies according to patient care plans and document patient response.
- Maintain accurate records and reports.
- Meet performance standards and participate in ongoing education and training.
Required Qualifications:
- Associate's or Vocational degree from a Board of Nursing approved LPN program.
- 0-2 years of nursing experience.
Preferred Qualifications:
- 0-2 years of acute care or home care experience.
Licenses/Certifications:
- LPN-MN-BN required.
- CPR - Allina Health required.
- Valid Driver's License required upon hire.
Estimated Salary: $65,000-$80,000 per year
