Vocational Case Manager Specialist

The primary objective of the Bi-Lingual Vocational Case Manager is to facilitate a seamless transition of clients into the workforce.
Key Responsibilities:- Meet with clients on an ongoing basis to discuss and support employment-related issues.
- Complete assessments, reassessments, transitional benefit plans, and care plans for active clients.
- Provide active referrals to internal and external providers to assist with job placement.
- Maintain and input data entry required by GMHC and other contracts, as needed.
- Complete employer site visits to support clients and continuation of care and retention.
- Assist with special projects and perform other related duties as assigned by supervisor.
- Bachelor's degree in Social Work, Human Services, Social Science, or Communications.
- Minimum of 3 years related experience.
- Bilingual in English/Spanish preferred.
- Excellent written, verbal, and interpersonal communication skills.
- Strong presentation skills with the ability to speak to large groups.
- Experience working with clients living with HIV/AIDS preferred.
- Intermediate knowledge of Microsoft Office Suite; Excel, Outlook, etc.
- Demonstrated experience in case management with diverse clients, with barriers to employment in jobs.
- Experience in working with current/former substance users in a social service organization performing case management services.
$45,000 - $48,000 per year
