Vocational Case Manager Job Description

The primary objective of the Bi-Lingual Vocational Case Manager is to facilitate a seamless transition of clients into the workforce.

Key Responsibilities:
  • Meet with clients on an ongoing basis to discuss and support employment-related issues.
  • Complete assessments, reassessments, transitional benefit plans, and care plans for active clients.
  • Provide active referrals to internal and external providers to assist with job placement.
  • Maintain and input data entry required by GMHC and other contracts, as needed.
  • Complete employer site visits to support clients and continuation of care and retention.
  • Assist with special projects and perform other related duties as assigned by supervisor.
Requirements:
  • Bachelor's degree in Social Work, Human Services, Social Science, or Communications.
  • Minimum of 3 years related experience.
  • Bilingual in English/Spanish preferred.
  • Excellent written, verbal, and interpersonal communication skills.
  • Strong presentation skills with the ability to speak to large groups.
  • Experience working with clients living with HIV/AIDS preferred.
  • Intermediate knowledge of Microsoft Office Suite; Excel, Outlook, etc.
  • Demonstrated experience in case management with diverse clients, with barriers to employment in jobs.
  • Experience in working with current/former substance users in a social service organization performing case management services.
Salary Information:

$45,000 - $48,000 per year
