Job Summary

We are seeking a highly skilled Advanced Practice Clinician to join our Home and Community Care team at Optum. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.

Key Responsibilities
  • Conduct comprehensive assessments and develop personalized care plans for patients with chronic conditions
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal health outcomes
  • Prescribe diagnostics and interventions to prevent unnecessary hospitalizations and promote cost-effective care
  • Stay up-to-date with the latest medical research, technology, and best practices to deliver evidence-based care
  • Participate in quality improvement initiatives and contribute to the development of clinical standards and guidelines
Requirements
  • Completion of a Nurse Practitioner or Physician Assistant program
  • Current state RN/NP or PA license
  • Certification through a recognized professional organization (e.g., AANP, ANCC, NCCPA)
  • Access to reliable transportation and ability to travel to patient homes
Preferred Qualifications
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Knowledge of CPT, ICD-10, and HCPC codes
  • Knowledge of clinical standards of care and NCQA requirements
