We are seeking a highly skilled Advanced Practice Clinician to join our Home and Community Care team at Optum. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.
Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans for patients with chronic conditions
- Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal health outcomes
- Prescribe diagnostics and interventions to prevent unnecessary hospitalizations and promote cost-effective care
- Stay up-to-date with the latest medical research, technology, and best practices to deliver exceptional care
- Participate in quality improvement initiatives and contribute to the development of clinical standards and guidelines
- Completion of a Nurse Practitioner or Physician Assistant program
- Current state RN/NP or PA license
- Certification through a recognized professional organization
- Access to reliable transportation
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of long-term care, community resources, and cost-effective alternatives
- Knowledge of CPT, ICD-10, and HCPC codes
