Job Summary

We are seeking a highly skilled Urology Nurse Coordinator to join our team at Yale New Haven Health. As a key member of our interdisciplinary healthcare team, you will play a vital role in coordinating and ensuring the implementation of patient care plans, utilizing the principles of case management.

Responsibilities
  • Establish a system for coordinating patient care throughout the continuum of care and linkage to resources through case management services.
  • Collaborate with inpatient nursing staff for coordination of complex admissions/discharges for gynecologic patients.
  • Collaborate with Primary Care Center and other community providers to facilitate women establishing primary care.
  • Collaborate with referring providers regarding treatment plans and needed follow-up.
  • Identify systems for follow-up for patients with abnormal pap smear results to ensure compliance with the plan of care.
  • Participate as a member of Women's Center team meetings.
  • Review healthcare information with the healthcare team, monitor patient progress, and adherence to clinical pathways or treatment plans.
  • Review care plans with providers, nursing staff, and other team members as appropriate, ensuring effective communication with patients.
  • Ensure that patients meet eligibility requirements and clinical needs for program services within established policies and procedures.
  • Collaborate with healthcare teams to initiate referrals to appropriate services and/or providers, ensuring accessible resources for patients.
  • Ensure that all patients with abnormal test results are case-managed within established policies and procedures.
  • Identify patients likely to have unmet insurance and resource needs, communicating with healthcare team members and other departments.
  • Review various reports regularly to ensure all services comply with established policies and procedures.
Requirements
  • RN, BSN required; Master's strongly preferred.
  • Minimum of five (5) years of relevant clinical experience, with at least two years in specialty areas of obstetrical and/or gynecological care.
  • Case management experience desirable.
  • Clinical credibility with physicians, nurses, and other healthcare team members.
  • Interpersonal skills to facilitate negotiation for scarce resources, facilitate changes in systems and/or practices, and broker with outside agencies.
  • Excellent communication skills to effectively communicate with multiple disciplines and skill levels.
  • Ability to quickly process complex problems, identify key components, and develop plans for resolution.
  • Effective interactions with all healthcare team members and the ability to elicit their cooperation.
  • Demonstrated skills in clinical management of specific patient populations, including working with and providing support to nursing.
  • Ability to provide clinical/financial education to colleagues and patients/families.
Accountability

Accountable for coordinating and managing the care of underserved gynecology patients, ensuring patients progress in accordance with established clinical and financial outcomes, intervening to correct delays, and having the authority to cross departmental lines.

Accountable for analyzing variances and implementing system and practice changes to enhance the efficiency and effectiveness of all departmental efforts related to case management.

Accountable for ensuring high-quality patient and family-centered care that is coordinated, efficient, and supports desired clinical and financial outcomes.
