Hospice Care Assistant

Job Summary:
We are seeking a compassionate and skilled Hospice Aide to join our team at Tufts Medicine Care at Home. As a Hospice Aide, you will provide personal care and support to patients in their homes, ensuring their comfort and dignity throughout their journey.
Responsibilities:
- Provide personal care and support to patients in their homes, including bathing, dressing, grooming, and toileting.
- Assist patients with daily living activities, such as meal preparation and medication management.
- Monitor patients' physical and emotional well-being, reporting any changes or concerns to the supervising clinician.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Provide emotional support and companionship to patients and their families.
Requirements:
- High school degree or equivalent.
- Certified Home Health Aide or Licensed Nurse Aide.
- Valid driver's license.
