Job Summary

Key Responsibilities
  • Coordinate and deliver primary health care and wellness services to patients, including obtaining medical histories, performing physical exams, and developing treatment plans.
  • Provide clinical and administrative assistance to programs, including health education, record-keeping, and statistical data analysis.
  • Collaborate with healthcare professionals to ensure seamless patient care and achieve clinic objectives.
  • Participate in quality review and improvement programs, monitoring quality and outcomes of care rendered.
  • Develop and maintain affiliation with professional and community organizations to stay current with best practices and industry developments.
Requirements
  • Master's degree in Nursing and current RN license in the state of Indiana.
  • Successful completion of a nurse practitioner program with applicable national certification.
  • Two years of clinical nursing experience in practice area and successful completion of a Nurse Practitioner six-month internship for new graduates or comparable experience.
  • Attainment of prescriptive authority or successful completion of applicable course work and pass state board exam to obtain prescriptive authority within six months of hire date.
  • Bilingual speaking and/or writing skills are a plus.
Working Conditions

This role requires the physical ability and stamina to perform the essential functions of the position, including walking/standing for prolonged periods, pushing carts/wheelchairs, lifting at least 50 pounds, and providing CPR.
