About the Role:

As a Care Manager I at CenterWell Home Health, you will be responsible for providing high-quality, patient-centered care to individuals in their homes. We are seeking an experienced and compassionate RN to join our team.

Key Responsibilities:

  • Provide admission, case management, and follow-up skilled nursing visits for home health patients
  • Administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager
  • Implement patient care plans in conjunction with patients and families to achieve optimal resolution of needs/problems
  • C coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers

Requirements:

  • Diploma, Associate, or Bachelor Degree in Nursing
  • A minimum of one year of nursing experience preferred
  • Strong med surg, ICU, ER, acute experience
  • Home Health experience is a plus

Benefits:

We offer a competitive salary range of $62,000 - $85,000 per year, depending on experience, plus a comprehensive benefits package that includes medical, dental, and vision benefits, 401(k) retirement savings plan, time off, short-term and long-term disability, life insurance, and more.
