Job Overview

Key Responsibilities:

  • Conduct assessments and develop comprehensive care plans for patients
  • Collaborate with other healthcare professionals to deliver high-quality care services
  • Document patient progress and report any changes to supervisors
  • Transport patient charts to their homes for signature as required by state regulations

Required Qualifications:

  • Certified Registered Nurse (RN) license in good standing
  • Previous experience in nursing preferred
  • Excellent communication and interpersonal skills
  • Valid driver's license and first aid/CPR certification

Estimated Salary: $65,000 - $85,000 per year
