Job Summary

The ALS Association is seeking a highly skilled and compassionate Care Services Manager to join our team. As a key member of our care services program, you will be responsible for developing and maintaining professional relationships with individuals living with ALS, their families, and caregivers. Your primary focus will be on ensuring the delivery of high-quality, consistent services that meet the unique needs of our clients.

Key Responsibilities
  • Develop and implement care plans that address the physical, emotional, and social needs of individuals living with ALS.
  • Collaborate with healthcare providers, social workers, and other stakeholders to ensure seamless care coordination.
  • Provide education, support, and resources to individuals living with ALS and their families, including information on services, benefits, and clinical resources.
  • Manage a portfolio of patients, ensuring that their needs are met and that they receive the highest level of care.
  • Work closely with the Managing Director of Care Services to evaluate the effectiveness of our care services program and identify areas for improvement.
  • Develop and maintain strong relationships with multidisciplinary clinics, healthcare providers, and community agencies to ensure access to resources and services.
  • Ensure that all care services are delivered in accordance with our organization's standards and policies.
Requirements
  • Bachelor's degree in social work, health administration, public health, nursing, or a related field.
  • Minimum of three to five years of experience in care management, case management, or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with individuals, families, and healthcare providers.
  • Ability to work independently and as part of a team, with a strong focus on customer service and satisfaction.
  • Proficiency in Microsoft Office Suite and experience with database management.
  • Ability to travel locally and nationally, as required.
