About the Role

The ALS Association is seeking a highly skilled and compassionate Care Services Manager to join our team. As a key member of our care services team, you will be responsible for developing and maintaining professional relationships with individuals living with ALS, their families, and caregivers.

Key Responsibilities
  • Program Development and Management: Develop and implement care services programs that meet the needs of individuals living with ALS and their families.
  • Relationship Building: Establish and maintain relationships with healthcare providers, community agencies, and other stakeholders to ensure seamless care coordination.
  • Case Management: Conduct comprehensive needs assessments, develop care plans, and provide ongoing support to individuals living with ALS and their families.
  • Education and Resource Navigation: Provide education and resources to individuals living with ALS and their families, including information on ALS, care options, and available services.
  • Collaboration and Communication: Work closely with the Managing Director of Care Services and care services staff to ensure effective communication and collaboration across the organization.
Requirements
  • Education: Bachelor's degree in social work, health administration, public health, nursing, or a related field.
  • Experience: Minimum of three to five years of experience in care management, case management, or a related field.
  • Skills: Strong communication and interpersonal skills, ability to work independently, and proficiency in Microsoft Office Suite.
  • Knowledge: Knowledge of ALS, care coordination, and community resources.
