Job Overview

We are seeking an experienced Nurse Practitioner or Physician Assistant for a community-based position in Lancaster and York counties, Pennsylvania.

About the Role

This field-based patient direct care position involves performing health risk assessments, depression screening, and addressing care gaps for Geisinger Health Plan Gold members. The successful candidate will be based in the patients' homes, with approximately 5-10% of visits conducted virtually through telemedicine.

Key Responsibilities:

  • Conduct comprehensive health assessments to identify potential health risks and gaps in care.
  • Develop and implement personalized care plans to address identified needs and promote optimal health outcomes.
  • Collaborate with healthcare teams to ensure seamless care transitions and effective communication.
  • Maintain accurate and up-to-date records of patient interactions and care plans.
