Medical Care Coordinator

Ascension Health is seeking a skilled Medical Caseworker to join our team. As a Medical Caseworker, you will play a vital role in coordinating the overall interdisciplinary plan of care for patients, from admission to discharge.
- Create a comprehensive plan for care across the continuum, integrating patient/family preferences and values.
- Assess, evaluate, recommend, and treat legal, neglect, substance abuse, crisis, psychosocial, and various adjustment needs.
- Maintain ongoing dialogue with supervisors and care transition team members to ensure effective implementation and reevaluation of health plans.
- Act as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses and treatment options for post-discharge care.
Requirements:
- Case Manager credentialed from the American Case Management Association (ACMA) preferred.
- Bachelor's degree in Social Work required.
