Care Coordinator

Your expertise will be essential in advocating for our members' needs and connecting them with the right services.
Key Responsibilities:
- Assess and develop individualized care strategies for our members, focusing on the most appropriate and least restrictive level of care.
- Identify and initiate referrals for member transitions to community resources, providing support with social services, financial assistance, and community programs.
- Manage the care plan throughout the continuum of care, serving as a single point of contact for our members.
- Communicate with stakeholders to ensure quality coordinated care and services are provided efficiently to our members.
- Advocate for our members and their families, ensuring their needs and choices are represented and supported by the healthcare team.

Requirements:
- A current, unrestricted RN license in the state of Texas.
- At least 2 years of experience working in a community health setting or a healthcare role.
- Intermediate-level computer proficiency, including Microsoft Outlook and Teams, and the ability to use multiple web applications.
- Reliable transportation and the ability to travel locally to visit members in their homes and other settings.
- Access to a designated quiet workspace in your home, with the ability to secure Protected Health Information (PHI).
Preferred Qualifications:
- Experience with long-term care services and support, Medicaid, or Medicare.
- Ability to create, edit, save, and send documents, spreadsheets, and emails.
- Knowledge of federal and state requirements, including the federal home and community-based settings regulations.
