Job Summary

The Housing Case Manager will work with clients who are receiving housing assistance to provide supportive services and help them remain stable in their new housing placements. The ideal candidate will have experience working with clients who have varying degrees of independence, mental stability, and/or are in active substance use, utilizing a harm reduction and/or Housing First framework.

Key Responsibilities
  • Build rapport with clients to identify their strengths and foster growth in self-sufficiency and resiliency.
  • Work with clients to locate, refer, and support them in acquiring needed resources, such as household items and personal needs, health resources, financial and employment resources.
  • Provide ongoing support and assistance as needed for newly housed clients in transition from housing instability to housing stabilization.
  • Advocate for clients with property management and ensure a successful housing placement.
  • Connect clients with health, mental health care, and substance use resources, as well as other supportive services.
Requirements
  • Bachelor's degree in a social services field or equivalent experience.
  • Two years of experience in direct social service or advocacy work, including assessing client needs and determining client benefit eligibility.
  • Proven experience working with marginalized populations, including chronic homelessness, past or current substance use/abuse, and ongoing mental health instability.
