About the Role

We are seeking a skilled RN Care Manager to join our team at Mercy. As a key member of our healthcare team, you will play a vital role in facilitating the discharge planning process from admission to discharge.

Key Responsibilities:

  • Collaborate with the healthcare team to ensure seamless patient care
  • Intervene with patients who have complex or high-risk psychosocial needs
  • Develop and implement effective discharge plans

Requirements:

  • Bachelor's Degree of Science in Nursing, required
  • 2-3 years acute care hospital setting experience, preferred
  • BLS (CPR) certification at hire date, minimum required
  • Certification in Case Management, preferred

Preferred Skills:

  • Strong verbal and written communication skills
  • Strong assessment, time management, and critical thinking skills
  • Ability to interact effectively with a variety of people and situations

Knowledge and Experience:

  • Knowledge of InterQual or Milliman Care Guidelines, preferred
