Job Title: Registered Nurse - Home Health

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Registered Nurse, you will work alongside an interdisciplinary team to provide high-quality care to patients in their homes.

About the Role:
  • Provide whole-person care to patients in need, working collaboratively with the healthcare team to develop and implement individualized care plans.
  • Conduct thorough assessments, develop and implement care plans, and provide education to patients and their families.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • Graduate of an accredited school of nursing.
  • 1 year of Full-time experience practicing as a Registered Nurse.
  • 1 year of recent (within last three years) clinical experience in a Medicare certified Homecare or Hospice agency/facility doing intermittent visits and case management.
