We are seeking a skilled Registered Nurse to join our team at Providence Home and Community Care (HCC). As a Registered Nurse, you will play a vital role in providing high-quality patient care in the comfort of our patients' homes.
About the Role:This is a full-time position working days, with a competitive salary and comprehensive benefits package. You will work alongside an interdisciplinary team to deliver whole-person care to patients in need, empowering them to achieve their best possible health outcomes.
Responsibilities:- Provide high-quality patient care in the home setting, working collaboratively with patients, families, and healthcare teams.
- Develop and implement individualized care plans, taking into account patients' unique needs and goals.
- Conduct thorough assessments, identify health risks, and develop strategies to mitigate them.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives, contributing to the development of best practices and evidence-based care.
- Graduate of an accredited school of nursing.
- 1 year of full-time experience practicing as a Registered Nurse.
- Current licensure as a Registered Nurse in the state of Washington.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
