Job Summary

We are seeking a highly skilled and compassionate Adult Family Care Case Manager to join our team at Advocates. As a key member of our care coordination team, you will play a vital role in ensuring the well-being and quality of life for individuals and families in our care.

Key Responsibilities
  • Act as a liaison between our organization, state agencies, and other service providers to ensure seamless care coordination and compliance with Medicaid regulations.
  • Process participant referrals, coordinate admissions, and facilitate the matching and transition of participants into homes.
  • Develop and maintain positive relationships with participants, families, and caregivers, and monitor and assess placements on a regular basis.
  • Train and supervise caregivers in all aspects of their responsibilities, including program requirements and best practices.
  • Work collaboratively with our AFC Registered Nurse as part of a multidisciplinary team to ensure comprehensive care and support.
  • Identify, monitor, and complete necessary assessments and documentation, including MDS/HC assessments and psychosocial evaluations.
  • Ensure accurate and up-to-date record keeping, documentation, and reviews, including daily logs, case manager notes, home inspections, and site safety plans.
Requirements
  • Bachelor's Degree in a related field, with a minimum of four years of experience, or Master's Degree with two years of experience.
  • Excellent communication and interpersonal skills, with the ability to develop and maintain strong relationships with employees, providers, and participants.
  • Intermediate computer skills and knowledge of Microsoft Office products.
  • Valid driver's license and access to an operational and insured vehicle.
  • Strong preference for candidates with experience working with culturally diverse populations.
