We are seeking a highly skilled and compassionate Adult Family Care Case Manager to join our team at Advocates. As a Case Manager, you will play a critical role in coordinating and providing high-quality supports to individuals, families, and providers served by Adult Family Care (AFC).

Key Responsibilities
  • Act as program liaison to the Executive Office of Elder Affairs, State agency Area Offices (DDS, MCB), and other Department personnel as delineated by Program Director.
  • Ensure that the program meets and complies with all applicable Medicaid regulations; oversee compliance of providers with mandated trainings.
  • Process participant referrals and the coordination of the admission process, as identified in State Agencies' regulations regarding "placement services".
  • Participate in recruitment, selection, and evaluation of Adult Family Care families. Interview families and facilitate the matching and transition of participants into homes.
  • Develop positive relationships with participants, families, and monitor/assess placements minimally on a regular basis.
  • Train and supervise families in all facets of their responsibilities.
  • Support the development and growth of all participants served in the program.
  • Work with the AFC Registered Nurse as part of the multidisciplinary team.
  • Identify, monitor, and complete certain sections of the MDS/HC assessment, as well as a psychosocial assessment.
  • Ensure and maintain that all program record keeping, documentation, and reviews are accurate and up to date.
Requirements
  • Bachelor's Degree in a related field with four years' experience; or Master's Degree and two years of experience.
  • Ability to communicate effectively verbally and in writing.
  • Intermediate computer skills and knowledge of Microsoft Office products.
  • Ability to develop and maintain satisfactory working relationships with employees and providers.
  • High energy level, superior interpersonal skills, and ability to function in a team atmosphere.
  • Must hold a valid driver's license and have access to an operational and insured vehicle.
