- Ensure appropriate follow-ups aligned with patient treatment goals, referring to external services as necessary.
- Work in conjunction with medical and food service teams to adhere to prescribed nutrition care protocols.
- Deliver education that complies with food safety and sanitation standards.
- Maintain accurate documentation of the nutrition care plan within patient medical records, detailing assessments, diagnoses, and progress.
- Uphold confidentiality regarding patient health information.

- Bachelor's degree required; Master's degree preferred in Dietetics or a related field.
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential is essential.
- Illinois state licensure as a Dietitian is required.
- Experience with PointClickCare and MDS processes is advantageous.
