Charge Nurse LPN Job Description

About the Role:

We are seeking a skilled and compassionate Charge Nurse LPN to join our team at SKLD Beltline. As a Charge Nurse LPN, you will play a critical role in providing exceptional care to our residents and ensuring the smooth operation of our nursing unit.

Key Responsibilities:

  • Assess residents' conditions and develop individualized care plans to ensure their physical and emotional needs are met.
  • Provide direct care to residents, including administering medications, performing treatments, and assisting with daily living activities.
  • Supervise and mentor Certified Nursing Assistants (CNAs) and State Tested Nursing Assistants (STNAs) to ensure they are providing high-quality care.
  • Collaborate with the healthcare team to coordinate resident care and ensure seamless transitions between shifts.
  • Monitor and report any changes in resident condition, including injuries, illnesses, or concerns.
  • Participate in quality improvement initiatives to enhance resident care and satisfaction.

Requirements:

  • Current state license as a Licensed Practical Nurse (LPN).
  • CPR certification.
  • One year of experience in long-term care or a related field.
  • Strong organizational and communication skills.
