Job Summary:

Our LPNs are responsible for ensuring the nursing needs of residents are met while actively incorporating our Philosophy for Person-Directed Care. You will work collaboratively with the care team to develop assignments, maintain a safe and healthy environment, and prioritize resident care and satisfaction.

Key Responsibilities:
  • Provide high-quality nursing care to residents, ensuring their physical, emotional, and social needs are met.
  • Develop and implement individualized care plans in collaboration with the care team.
  • Conduct regular assessments and evaluations to ensure resident safety and well-being.
  • Collaborate with the care team to ensure seamless transitions and continuity of care.
Requirements:
  • Current LPN/LVN license in the state of employment.
  • Current CPR certification.
  • Prior nursing experience in senior care is preferred.
