Job Description: Physical Therapist
We're seeking a compassionate and motivated Physical Therapist to join our team. As a Physical Therapist at JAG Physical Therapy, you'll be responsible for assessing patients' needs and creating personalized treatment plans to aid in their recovery and functional independence. This role requires maintaining patient confidentiality and collaborating with a healthcare team while providing effective interventions and education to patients.
Your Responsibilities:
• Assess patients' strengths and weaknesses to develop creative and effective treatment plans
• Collect and record patient care data, including initial evaluations, daily notes, reassessments, progress reports, discharge plans, and communication with members of the healthcare team
• Prepare for evaluations by obtaining patient history, researching unfamiliar diagnoses, and selecting and administering tests and measures
• Synthesize evaluation results to formulate diagnosis, goals, prognosis, and plan of care, and establish treatment plans based on evaluation results, goals, and best practice
• Collaborate with interdisciplinary teams, patients, and patient support systems to assess and re-assess the need for modifications to plans of care and goals
• Identify the need for consultations and/or referrals to other healthcare professionals or services, and initiate and complete discharge plans
• Provide education/training to patients and/or patient support systems on ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription and training, and community reintegration
• Provide interventions to patients that are safe, effective, and in compliance with JAG guidelines
• Oversee support staff, including PTA, ATC, and PT aides
• Attend annual JAG Billing and Coding seminars and promote JAG in the community through lectures/presentations
Requirements:
• Bachelor's or Master's Degree required; Doctorate degree preferred
• Licensed Physical Therapist in the state practicing in
• Completed state-regulated CEU requirement in appropriate state (NY, NJ)
• Valid CPR License
• Experience/knowledge with an EMR software preferred
• Excellent oral and written communication skills
• Strong clinical decision-making skills
• Excellent work ethic and dedication to patient success
• Basic computer skills
• Ability to work as a team member and individually with minimal supervision
• Positive, professional demeanor at all times
