Job OverviewThis role offers a unique opportunity to join our team as a RN Transfer Center Team Member - Patient Logistics Specialist. In this position, you will play a critical role in facilitating patient transfers into SSM Health from other healthcare facilities, physician offices, and clinics. Job Responsibilities- Facilitate, track, and document all hospital transfers within the Transfer Center
- Respond promptly to phone calls, demonstrating exceptional customer service skills and maintaining composure with challenging callers
- Collaborate with relevant facilities based on services required, geography, facility capacity/capability, and obtain necessary medical and administrative approvals
- Follow established protocols, policies, and utilize resources to ensure program requirements, including regulatory requirements for Emergency Medical Treatment and Labor Act (EMTALA), are met
- Communicate patient information and estimated arrival times to prepare physicians and facilities for patient admission and placement, working collaboratively with patient placement and other team members to implement plans of care
- Utilize problem-solving skills and department processes to determine the best course of action that meets the needs of patients, physicians, and hospitals
- Maintain accurate databases of transferred patient information, accurately documenting and monitoring activities from initial physician referral to inpatient admission and patient placement
- Graduate of an accredited school of nursing or education equivalency for licensing
- Five years of registered nurse experience in an acute care hospital setting
- Familiarity with Transfer Center operations and patient logistics
- Excellent communication and interpersonal skills
- Able to work effectively in a fast-paced environment
