Per Diem Hospice Aide / Certified Nursing Assistant

The hospice aide is a vital member of the interdisciplinary team, working under the guidance of a registered nurse to provide essential services to patients, ensuring their personal needs are met and promoting overall comfort. This role involves careful observation of the patient, reporting findings, and documenting care provided. The hospice aide is assigned in a manner that ensures quality, continuity, and safety in patient care.

KEY RESPONSIBILITIES
  • Provide personal care based on patient assignments and respond promptly to requests.
  • Ensure patient safety and use equipment properly (e.g., foot stools, side rails, oxygen).
  • Deliver personal care services such as bathing, back rubs, oral hygiene, and changing bed linens as required.
  • Assist patients with dressing and undressing as needed.
  • Plan and prepare nutritious meals, including shopping, as assigned.
  • Support patients during feeding as instructed.
  • Measure and record vital signs, including temperatures, pulse, respiration, and blood pressure, as ordered.
  • Monitor and care for the patient's skin to prevent tissue breakdown.
  • Report any changes in the patient's condition to the Case Manager and be aware of interpersonal dynamics involving caregivers and family members.
  • Assist with ambulation and exercises as directed by the hospice nurse or therapist.
  • Provide assistance with bedpans and urinals, and help with light laundry tasks as assigned.
  • Perform range of motion exercises and other simple procedures as directed.
  • Offer respite care for patients and their families when on-site, as appropriate.
  • Maintain a clean and orderly living environment for the patient, as assigned.
  • Adhere to the organization's documentation standards and professional conduct guidelines.
  • Participate in quality assessment and performance improvement initiatives.
  • Respect the personal needs and environment of patients and their families.
  • Note: The hospice aide will not engage in any activities considered nursing practices as defined by the Nurse Practice Act, including administering medications or performing sterile techniques.
This summary represents the primary duties and responsibilities of this position. Additional job-related tasks may be assigned as necessary.

QUALIFICATIONS (Education, Experience, Knowledge, Skills & Abilities)
  • Completion of a state-approved training program and competency evaluation, or state licensure program.
  • Meets state training requirements.
  • Must be at least 18 years old.
  • Ability to read, follow written instructions, and document care provided.
  • Understanding of hospice philosophy and comfort in providing care to terminally ill patients.
  • Positive references from previous employers.
  • Self-directed with the ability to work independently and manage issues related to death and dying with composure.
  • Empathy for the needs of the ill, injured, and frail individuals.
  • Current CPR certification is required.
  • Exhibit patience, tact, and maintain good personal hygiene.
  • Valid driver's license and access to an insured vehicle in good working condition.
  • Strong oral and written communication skills.
  • Good organizational abilities.
